No. A PO Box is a USPS-only address that starts with "PO Box." Yours is a real street address at 2082 HWY 183, Ste 170, with your own suite-style number. UPS, FedEx, and DHL will all deliver to it.
Two forms of ID, one of which must be a government-issued photo ID such as a driver's license, passport, or state ID. The second can be a voter registration card, vehicle registration, lease, or recent utility bill.
It's the federal form that authorizes us to receive mail on your behalf. It has to be notarized — and since we have a notary on site, we handle it for you while you're here.
